Dear Claimant:
This form will allow you to begin the process through which you may be able to recover money for city related damages you have suffered. Before completing the form, however, you should be aware of a few basic facts which will both clarify the process and increase your chances of filing a successful claim.
Please be advised that it is essential to complete the form in full. All the information requested is necessary to fully and fairly consider your claim. It is particularly important to remember that the law requires you to type your answers on the claim form. You should also be sure to include two written estimates of the repair claimed damage (or a copy of the paid bill for the repairs you have made) as well as a copy of the police report on the incident.
After we have received your typed, completed claim form, the city clerk's office will introduce the claim, on your behalf, at the next meeting of the Chicago City Council. At that time, your claim will be referred to the Council's Committee on Finance. The Committee will investigate your claim and on that basis, report back to the City Council with a recommendation that the Council accept or reject your claim. You will be notified of the Council's decision by the Committee on Finance.
