Freedom of Information requests should be printed or typed and include the following information: the name, mailing address and daytime phone number of the requester; and a description of the records being requested. Requests can be mailed, faxed or dropped off in person. If dropping off in person, please contact Cristina Franzoni at 312-744-2507. Requests can also be submitted using the following form:

The City Clerk’s Office maintains the following categories of records:

      • Official records of city government
      • Publishes Journal of Proceedings of the Chicago City Council
      • Business, liquor and other city licenses
      • Kids ID and Medical ID records
      • Vehicle sticker and residential permit parking sticker records
      • City Clerk’s Office and City Council personnel files and payroll records
      • Marketing materials including press releases and brochures
