The Office of the City Clerk will now handle Automatic Amusement Device Licensing. As of December, existing license holders will be able to update device information, make payments, and receive their licenses from our office.

Since our office is providing this service, the forms are slightly different this year. Instead of filing a separate return for the gambling format and non-gambling format devices, you can now complete one return for both formats. Also, there is additional space for device information on the forms. Although our office has your device information from previous returns, since the information was not consistently updated, we have sent out blank forms this year. We will input the information you provide into our database and in subsequent years you will receive a pre-filled license registration form with the updated device information.

You can file this form by mail or in person. The return address for mailing is:

However, since you are submitting information from scratch, we recommend filing in person at the Office of the City Clerk, Room 107 in City Hall. Please note that the Office of the City Clerk cashiers are located directly across from the Department of Revenue cashiers. In this way we can ensure that your information is correctly inputted and verify the correct amount for your payment.

To avoid late fees and penalties, your completed license registration form and payment must be received or postmarked by the United State Postal Service on or before February 15th. Only forms provided by the Office of the City Clerk through a mailing or linked to below will be accepted. Unsigned or incomplete forms will be deemed filed late and will be assessed applicable penalties. Please write your account number and the tax period on your check or money order.
