| Chicago Medical I.D. Program
Para la información médica de identificación en español, haga click aquí. The Chicago Medical I.D. program offers a free medical identification card to Chicago residents of all ages who want their medical information and vital statistics readily available in the event of an
emergency where they might become incoherent or experience confusion. The front of the identification card will include the following information: Chicago residents can apply for a Medical I.D. card at our City Hall office by appointment only until further notice (121 N. LaSalle St. - Rm. 107) or at our satellite facility by appointment only until further notice (5301 S. Cicero Avenue, Ste. 104). Our City Hall office will provide this service on Monday through Friday, 8 a.m. to 4 p.m. and our satellite facility is open Monday, Tuesday, Thursday and Friday, 8 a.m. to 5 p.m. and on Wednesdays, 8 a.m. to 7 p.m. (closed Saturdays).
