Chicago Medical ID Application (PDF)

The Chicago Medical I.D. program offers a free medical identification card to Chicago residents of all ages who want their medical information and vital statistics readily available in the event of an emergency where they might become incoherent or experience confusion. The front of the identification card will include the following information:

       • Name
       • Address
       • Date of Birth
       • Emergency Contact Name
       • Phone Number
       • Photo

On the reverse side, the card will include the following information:

        • Primary Care Physician Name and Phone Number
        • Medical Conditions
        • Blood Pressure Status
        • Medical Allergies

In order to participate in the program, Chicago residents must provide a form of identification.
Participants will have their photo taken and personnel from the Chicago City Clerk's Office will merge the photo with the descriptive information and contact information to generate the ID's for
individuals to carry with them. The medical identification card will be mailed two-to-three weeks
later. To schedule an on-site program for groups of 50 or more participants, contact the Special Projects Division of the Office of the City Clerk at (312)744-2506.

Chicago residents can apply for a Medical I.D. card at our City Hall office by appointment only until further notice (121 N. LaSalle St. - Rm. 107) or at our satellite facility by appointment only until further notice (5301 S. Cicero Avenue, Ste. 104). Our City Hall office will provide this service on Monday through Friday, 8 a.m. to 4 p.m. and our satellite facility is open Monday, Tuesday, Thursday and Friday, 8 a.m. to 5 p.m. and on Wednesdays, 8 a.m. to 7 p.m. (closed Saturdays). To schedule an appointment, please contact the Community Services division of the Clerk's office at 312.744.2506.
